What is Support Coordination?
Support Coordination, also known as Case Management, is the process of arranging and monitoring services for a participant and their family. Support Coordination provides participants and their families with the assistance of paid professionals who are responsible for providing the necessary information to ensure access to all available services, as well as to aid in making an informed choice about said services.
