Qualifications
A Support Coordinator must meet the following requirements:
- A bachelor’s or master’s degree in a human service-related field such as:
- Psychology
- Education
- Counseling
- Social Services
- Sociology
- Philosophy
- Family & Participant Sciences
- Criminal Justice
- Rehab Services
- Substance Abuse
- Gerontology
- Vocational Rehabilitation.
Defined Responsibilities
The Support Coordinator will be responsible for completing all of the following required components and any other duties deemed necessary by Supervisor and Program Director.
- Intake Need: Determination of eligibility and management services.
- Assessment: Process of gathering and integrating formal/informal information concerning a participant’s goals, strengths, and needs to develop a comprehensive/individualized service plan.
- Service Planning: Development of a written agreement based upon assessment data, observations, and other sources to reflect the participant’s needs and priorities, specifying the services and resources required to meet those needs.
- Linkage: The actual implementation of the service plan, which involves arranging for a continuum of formal/informal professional services to be provided to meet the participant’s needs.
- Advocacy: Assurance that the participant receives appropriate services of high quality and locating additional services not readily available in the community.
- Follow-up/Monitor: Regular communication with the participant’s professionals and service providers to determine that the service plan is effectively meeting the participant’s needs.
- Re-Assessment: Review of the current plan through interview and observation to analyze progress towards accomplishment of goals in the overall service plan.
- Transition/Closure: Discharge/closure must occur when participants no longer need, desire, or participate in the case management process or become ineligible.
- National Voter’s Registration Act: Support Coordinators will offer each participant a voter’s registration form at all initial assessments, reassessments, and when a participant has a change of address. Assistance with the completion of this form will be provided if needed. Completed forms will be submitted to OAAS per current policy indicates.
